Not many things in the game interact with you when you have no effects out. An object that you don't need an effect for HAS to have a raisin. So you flick it, hoping to get another effect when suddenly UBOAAA.

It's scary because it's something that happens so suddenly in a rather quiet scene. Also, you can kill that blonde girl and he'd still appear.
It's just something you should have played without knowing about him to understand how scary he is. If you already know he'd show up, it loses all of its frightfulness.
