I've seen this before, some blood is drawn but the stab missed completely; it's just lag/desync.

You might have been on one of my servers. I have been testing overheal lately and forgot to make backstab do more damage.

Scout, medic, engie, sniper, and spy are the only classes backstab was not possible on.
